We use SimplePractice for our group, supervision, and consultation registrations.

However, their platform is a bit limited on the initial sign up, so please follow these instructions:

1. Click on the link below (it will take you to the "new client" form)

​

2. Enter your information. In the "is there anything else you want to share" field, please enter the name of the group or type of supervision you're registering for. 

​

3. Once I receive your request, I will send you a registration form via Simple Practice.

 

4. Once you complete the forms, I will either schedule you into the group or contact you to discuss scheduling.
